Intravenous volume replacement: which fluid and why?

چکیده

Fluids available for intravenous volume replacement may be either crystalloid or colloid. The fundamental differences between these fluids are their effects on the Starling equation (table 1) which describes fluid flux between the intravascular and interstitial spaces. Starling stated that the rate of fluid movement into or out of a capillary is related to the net hydrostatic pressure minus the net colloid osmotic pressure.1 The Starling equation has been modified to include coefficients which represent the permeability of the capillary membrane to small solutes (Kfc) and its ability to prevent large molecules such as plasma proteins from crossing it (oc).' Colloids may be used to replenish the oncotic strength of the blood, thereby enhancing its water retaining capacity.

Volume replacement in infants with dengue hemorrhagic fever/dengue shock syndrome.

Volume replacement was studied prospectively in 208 infants with dengue hemorrhagic fever/dengue shock syndrome (DHF/DSS). The mean volume of intravenous fluid used was 110.4 mL/kg administered over a mean period of 25.8 hours. The mean volumes of intravenous fluid replacement in infants with DSS was significantly higher than in those with non-shock DHF (129.8 mL/kg versus 102.1 mL/kg; P = 0.00...

Fluid therapy for the surgical patient.

Perioperative fluid therapy is the subject of much controversy, and the results of the clinical trials investigating the effect of fluid therapy on outcome of surgery seem contradictory. The aim of this chapter is to review the evidence behind current standard fluid therapy, and to critically analyse the trials examining the effect of fluid therapy on outcome of surgery. The following conclusio...

Intravenous fluid replacement minimised dehydration during sodium picosulphate bowel preparation for colonic surgery.

Intervention 19 patients were allocated to receive a calculated volume (mean 2 l) of intravenous normal saline (treatment group), and 22 were allocated to receive no intravenous fluid (control group) during bowel preparation. Intravenous fluid administration (hourly infusion rate 4 ml/kg for the first 10 kg, 2 ml/kg for the second 10 kg, and 1 ml/kg for each subsequent kg) began at the same tim...

Prevention of Contrast-Induced Acute Kidney Injury: Is Simple Oral Hydration Similar To Intravenous? A Systematic Review of the Evidence

BACKGROUND Pre-procedural intravenous fluid administration is an effective prophylaxis measure for contrast-induced acute kidney injury. For logistical ease, the oral route is an alternative to the intravenous. The objective of this study was to compare the efficacy of the oral to the intravenous route in prevention of contrast-induced acute kidney injury. STUDY DESIGN A systematic review and...
